My guess is it was clearly planned. Like Brain said, why would it be so easily detachable. Just look at the closeup pic that Rape provided. The outfit had button snaps on the breast area.

I don't know if it was intentional or not, but is it coincidence that the line in the song as it was happening was "have you naked by the end of this song"? And Justin's hand just happened to be on her boob at that exact moment.
